Care Coordinator/Case Management Full Time

Case Managers support older Vermonters in the community to stay as independent as possible in the environment of their choice by promoting health, rights, independence, and economic well-being.

This position is based in one of our three offices (Barre, Morrisville, Randolph), combined with in-home field visits. Staff are never asked to be on-call or work weekends.

• Pay Range: $24-$26 per hour. Generous benefits including retirement plan, medical, vision, and dental insurance, and paid time off.
